What is difference between a survey and a psychological questionnaire?
A survey is used by researchers to gather information about a particular group of people, for example what people earn or for what party they vote. The questions in a survey ask directly what people want to know: “How much do you earn each month?
 
A psychological questionnaire differs through asking questions about something in order to measure something else. An intelligence test does not ask “What is your IQ?”. Through asking questions about your behaviour, for example, psychologists have an idea about your personality.
